Just found the answer to my question above.  In order to show the delete action ( or any other action) I had to go on the web form step and check "enable entity permissions"

Thank you.  Yes, I didn't have the entity permission set for the Notes entity.  As soon as I set it to the web role, it worked.
Now, I have another issue.  I am trying to allow the portal user to delete a note.  I went under "metadata" on the webform and added a record for the Note to allow delete.  However, it doesn't show anything different.  I can view the files but can't delete them.

    Hi Ashish,

Kindly provide the entity permission for Note(Association) entity to the associated logged in contact user and even for the anonymous user.

    Thanks!

one more observation is you should create Entity permissions for Note entity(chilled) and as well as parent Entity.

I am also having the same issue.  The file displays find in Dynamics on the entity form but on the portal, I get "Not Found" when I click on the file link.
