Need to change contact form title

I have a requirement to change the Contact Form title to concatenate name + (ID) and in some cases I need to put in an Alert too. 

I am tried using H1 tag of class .ms-crm-TextAutoEllipsis but it is not working.

Has anyone done that yet?

    Hi Montek,

    Can I just clarify - you are just wanting to change what is displayed on the form when it is opened (not actually change the underlying data held)?

    Perhaps try this link from Inogic:

    Controls contained in the header can be accessed in CRM 2015 using the convention "header_" to prepend the name of the control. For instance, if the 'name' attribute is in the header of the form, you can access it using this :

    Crm shows primary attribute value in form title.. for contact fullname is shown in form header..

    easy way to concatenate id is you need to add id in contact nmae..

    But this will change  data..

    Then another option is unspported way to change form header..

    The Contact Full Name is what's displayed as the form title. Don't think this is customizable. Instead, how about displaying a Form-Level notification? You can do that through JScripts.

    This Contact title will be retrieved through the Contact name as the primary key by default.

    I was just searching for similar information to the OP and came across this:

    crmxpg.nl/.../change-form-title-in-ms-crm-via-javascript

    I've not tried this code so I can't verify that it works.  Also, I suspect this code is not supported by MS which is why I won't be using it myself.  However, I haven't seen any other solutions suggested online.
