Business Process Error question

Our user community received this error last night for a brief time and could not create cases.  Eventually the system came back to normal and users were able to create cases.....any idea what may have caused this process error?

    Sounds like you have a third-party(or, maybe, an internal) solution, and there is a plugin that communicates to a web service (the plugin name is ICS.PharMerica...)

    From what I can see, that web service was down. Then it probably just came back online..

      it's, basically, the same, just a bit more details.. There is an error from the plugin:

    Unexpected exception from plug-in (Execute): ICS.PharMerica.Plugins.AccountLastCase: System.ServiceModel.CommunicationObjectFaultedException: The communication object, System.ServiceModel.Channels.ServiceChannel, cannot be used for communication because it is in the Faulted state.

     Everything works fine, even the plugin, it seems. However, the plugin is trying to open a connection to some web service (you can't say which one just by looking at the log - will need to look at the plugin code), and that part fails.

     Here is the plugin name:

    ICS.PharMerica.Plugins.AccountLastCase: ICS.PharMerica.Plugins.AccountLastCase
