Notes entity attachment not work

Hi,

I am new in microsoft dynamics CRM .

I create a new free trail instance of CRM and reset the instance.

Now, i am using annotation (notes) entity to custom entity form but i can't attach the file to notes.2055.test.png

    Try to create that particular record before trying to attach a file to it.
