Lead qualification button is working only from the lead list page, but not from the form. tried creating new form and filling all mandatory information

Could you please elaborate on this? Is there any customization? Or is it the OOB entity?
If it the OOB Lead entity, the required fields are last name and topic. Once you add data to those fields and save, it is possible to qualify the lead.
If you provide company name and then try to qualify then it would work.

There was some customization, but now i have removed it but still it is not working. Dont know what customization is still left

What do you mean by not working, does it give any error? If yes, please post the log.

by not working, does it not do anything or are you experiencing an error?
The customization were new javascripts to enable and disable certain controls on the lead form, some other UI based changes. Can i revert back to original lead entity?
This doesn't give any error but infact the button do nothing. I have tried to use the ribbon workbench it gives some weird error when i try to publish the lead entity

The Ribbon XML is faulty, you need to use Ribbon Workbench to see where the issue is. Make a new Solution and just add you Lead Entity to it.
Use Ribbon Workbench to Launch the Solution and investigate for the Qualify button.
Also, you can bring in your Lead Entity from the Vanilla CRM Organization, if you really want to replace it.
You can use the Online Trial for that. Make a Solution and add the Lead Entity without dependencies. Export as Unmanaged Solution and then import it to your Organization then publish the Solution.
In this way you can get back your original Lead Entity (you will loose all your customizations).
