Account name cannot be added in CRM. is that by default or any changes should be done

No, this is not by default. This must be a customization - field is disabled on the form by configuration, JS, Business Rule
Check your solution for either form control settings: Control is Read Only, Business Rules or JavaScript.
Somewhere in there your control is set to read-only/disabled.

Go to the form editor as shown below:
and double click on Account Name field as shown below:
and unchecked this option as shown below:
I think this will resolve your problem.
