I know CRM 4 is very old, and I think it's unlikely, but I have been asked whether it is possible to integrate Adobe E-sign with it.
Does anyone know?

CRM 4 is not supported by Adobe. You can refer below link

I thought it could maybe be integrated via the API?
In this case, would be good if you can directly contact Adobe. I believe they can provide better answer in this case.
