Blank Record in Approval User Setup

Hi,
 
Did anyone encounter a blank record suddenly created in Approval User Setup, as shown on the screenshot below? SystemCreatedby was shown as 00000
 
BC Version:
 
This has happened in 2 different environments and it's highly unlikely was created incidentally by user.
