I kept getting this error when I started a new custom report for a client. 
Turns out the connection string "disco.crm4.dynamics.com;<organizationname>" was my problem.

After some trial and error I created a custom report by using the wizard in crm and downloaded that report. The connectionstring contains the organization UNIQUE NAME instead of the organization name.
For those who do not know where to find the organization unique name:
CRM > Settings > Customizations > Developer Resources

Is this always the case in CRM 2016? For another client still on CRM 2015, the organization name in the connectionstring still works.

    Fair point.

    Quickly checked one, and there they were indeed equal.

    Must have been very lucky up until now. :-)

    Are you sure that for previous customers you mentioned actual and unique names were different and you was able to use actual name?

    Before today, I never had an issue in using the actual organization name in the connection string.

    Even today, I work for customers in CRM Online 2015 where I can use the organization name in the connection string.

    Just for this new user I now have to use the unique name "org1234" instead of the organization name.

    When the CRM Url is: orgname.crm4.dynamics.com

    The connectionstring has always been: disco.crm4.dynamics.com;orgname

    Now I had to change this to: disco.crm4.dynamics.com;orguniquename

    Where the orguniquename is not equal to the orgname.

    I found it a bit strange. :-)

    Fairly it worked that way since FetchXml reports were released in CRM 2011.
