Synchronizing data information on a third party app with crm

I have a scenario in mind, and would need help to the answer.

Here it is:

Say you have an app that is used capture customer's basic info (name, D.O.B., sex, marital status,....those kinda things), and you want that app to sync with CRM to register the basic info of that customer's details upon hitting the 'Submit' button on the phone. How can one achieve that.
