Regarding MenuItem referenced for a form

Hi team,

I am trying to do a refund operation, but when I click on the refund button it is throwing the following error "No object specified on menu item abc_menuitem".
I checked the code for this particular form, it is referring to the correct menu item. Even in menu item property object type & object is selected correctly. 
Still getting this error, it would be helpful if I get any inputs from you all.

    I can think only three reasons.
    1) build/synch not well done. So redeploy the code and make sure database gets synchornized.
    2) Debug to see where failure occurs. the point of failure may be further down the line, beyond the menu item, in some class or form.
    3) if it is not a customized form, simply raise a support call with Microsoft.
