Unable to Display SQL Report on Dynamics 365 Form Section

I have an SQL report that I need to display inside a section of a Model-Driven App form in Dynamics 365. To achieve this, I am using an HTML Web Resource with an IFrame component.

Below is the code I am using:

 

Issue:

When I try to load this in the Dynamics 365 form, the report does not display. Instead, I encounter the following error:
