Unwanted lines displaying on Customer Insights Journey form embedded on our website

Hello everyone
 
We're using the embed form option for a very simple form which contains the customer's email address (read-only) and a tick box.
 
When I publish the form using the standalone option, it looks fine:
 
However, once embedded on the webpage, there are five unwanted horizontal lines.
I can't find anything in the form HTML that causes these to display and I wonder if anyone else has experienced something similar?
