As mentioned by André and Sukrut; you have to go for customization to achieve this.
Before the start; please confirm with the users, the area where they will maintain the attachment in the document. This could be in header or in lines.
Once this is confirmed; you can do the following.
--- Display either a popup message or show a notification bar like "Attach a file is mandatory" wherever a new document is created in system. Set this validation at the initial stage of document creation. This is important because, the document could have linked with the workflow approval process. Therefore; system should validate this first to ensure, without attaching any document, you cannot proceed further.
--- At this stage; you have to disable both Lines and the Journal batch number (which usually creates as a link to click) options in the journal so that users should not be able to move forward. In case of PR or PO, you can simply set this validation just after the PR or PO is created.
--- Once the users attach the file in the 'Attachment', then only system will allow them to proceed with the document.
