GP Workflow Document Status Set to Error Occured

Hello,

We are running into random errors on certain workflow documents status changed to Error Occurred by System Account around the same time. Is this related to permission on document list? This is weird error since we had lot of workflow document completed successfully. Any idea what might be the issue?
