Background workflow will not run as a Real Time Workflow - Dynamics CRM

Hi Guys
 
I have a really simple workflow (see below) - this runs absolutely fine as a Background Workflow - but will not run when converted to a Real-Time workflow. I am System Admin so have the /Activate Real-time processes/ and /Execute Workflow job/ privillages. So I am at a loss why it won't run. 
 
All it does is automatically resolve an Enquiry (Case) when the user ticks a 'Resolve Enquiry' tick-box when sending an email:
 
 
Any ideas/advice would be greatly appreciated.
