Reset last used form

To test a new workflow and business rule, I made a temp copy of a custom form for the account entity and used it for a little while.  After I was done, I implemented the business rule in the original custom form and deleted the temp copy.  

Now, whenever I try to view an account, I get "Record Is Unavailable - The requested recod was not found or you do not have sufficient permissions to view it."  I've disabled both the business rule and the workflow to no avail.

I suspect the problem is my user record is still pointing to the last used form, which is the temp copy I deleted.  How can I have CRM switch my account to use another form? It doesn't seem to get passed the form not being there and cascading down to the next down the list.

I'm using CRM Online 2015 and I'm a system administrator.

    Make sure that one of the form of the entity is set for "Enable for fallback" so that user can use it to see the record.

    Thanks.  I have two forms (the OOB and a custom one) set for fallback.  No juice.

    try deactivating one of these main forms. if the problem persists, I advice you to ask for microsoft help.

    "Last used form" should only persist during a session. At next logon you should get the first listed form that you have access to through your roles.

    Have you tried signing out, closing all browser windows and signing back in?

    (I know, this is really saying "turn it off and on again", but for good reason!)

    I had same problem today. I think I found solution:

    1) Open from entity form which you like to open to design

    2) Open that form to new window (you get access to url)

    3) Copy from url string "formId%3d1932E0F9-78CE-4B50-B7EE-F18C65925842%26" (it mean formId=GUID&)

    4) Open record of entity , get its url.

    5)Add to url after "extraqs=" "&formId=GUID&"

    6) Open that url. Now you have fixed it.

    I believe Adam Vero's is the correct answer!  Thank you for your suggestion, Adam.

    I created a new user with sys admin rights and logged in as that user on the same machine with the same browser (latest version of Chrome) and the new user had the same problem.  So the last form used setting does not appear to be stored online.

    I then logged in from a different computer with a different browser and did not have the problem.  When I logged into the original machine after completely shutting down the browser I did not have the problem.

    So, it does seem to be session only.

    I decided to research a bit.  There is a cookie that goes by the name of the CRM organization.  Among a couple of settings, it sets up Local Storage.  Within that Local Storage I was able to find specific, human-readable references to default forms for entities.  See the image I've included.

    To test, I deleted first just the Local Storage and then the entire cookie, but without completely shutting down the browser (had other important things going on).  However, after signing out of CRM, deleting the cookie, and re-signing in, CRM still knew which form was last.  I suspect that's because I didn't shutdown the browser and the setting may have still been in memory.

    Bottom line, if you have this issue, shutdown the browser completely and, just in case, delete (as a minimum) the CRM org cookie.
