You are getting is because the record you are looking up has a name field with no values. I recommend that you create a flow or workflow that generates the name on creation.

In the form editor, go to the properties of the field called "Client Account Name", and under Details, see what the field name (not the display name) is.  Now click on the entity in your solution (or the default solution) and see what field is listed as the Primary Field.  Do those two field names match?

    Tiffany,

Can you please click on that contact or account, navigate to form and post a screenshot here - I interested in Name field for Account and Full Name field for Contact. Can you also post a screenshot of header of those forms. If display name is available then you have something that breaks display of proper values. Do you have by a chance onload javascript that touches those fields?

    Hi,

Agree with above suggestion. The easiest way to check this is by clicking on that record (no name) and check if it has any value in the name field. You then need to identify the cause of this record being no name.

    Hope this helps.

If the display name field of that record is empty, then you will experience this. Make sure data is entered in that record name attribute.

By default that field is mandatory to enter, sometimes people customize to append or capture some auto-number value in plugin, etc. That might be broken. Check it.
