When I've had this problem, it was because I removed "Company Name" from the Lead form. Adding the field back fixed the problem with the Qualify button.

Please check the following when working with the qualify button on the leads form, that does not work:
• Check to see whether this is happening just on your user profile or across the organisation
• Log in with the default system administrator account, if you can get access to it, and check to see whether the problem is with this account as well
• See if there are any customisations / developments that have been created that maybe stopping the qualify button to execute. This includes workflows, actions and in the background.
• Check your browser, is this happening in one browser or another. The most stable browser to test generally is Internet Explorer.
• Check your internet settings on your browser
• Check your pop ups on the internet browser, set them to allow
• Recreate your default web form
• Are there any duplicate detection rules stopping the qualification? If so, remove the duplicate detection rule for leads for now.
• Check to see whether this record exists in accounts and contacts already, this is just for tracing proposes
There has been a reported bug from Microsoft that states there is a fix coming for the qualify button. If it is this, then: Last update from the 23rd: "Deployment of the fix has been delayed, but is expected to be completed by Friday, July 7, 2017."

The qualify button on the lead form does nothing when clicked. This has been an issue for two weeks. This should be a known issue as it has been reported elsewhere in forums.
