I just create new trial of dynamics 365 online, and want to create new custom form for phone / tablet, but there is no option to create form for phone / tablet?

Just had a follow-up question to this thread. I'm still unsure how we can create one form for Mobile and one for Desktop, and I can think of many cases where this would be needed. We have some functionality we only want our users to do on a desktop, plus if we have three columns of fields we would put the fields we want with most visibility to be at the top of each column as to be at the top of the page -- but in Mobile view these would load on top of each other with the top fields in the second and third columns being way down the page. Also are there any limitations with using CRM 8 with the latest mobile app? I keep reading about Unified Interface, but all the tutorials about this seem to be geared around CRM Customer Engagement v9 and not v8. Thanks.

You can design separate form for mobile users and desktop users and make it work by setting form order and enabling security roles but I would recommend going with the default out of the box one main form for all devices approach. With this approach, you will save a lot of time in the long run. You only have to make changes to one form. and You only have to test one form for all different client types.
When you design as just one form, you could take advantage of the form element property to hide a certain area of the form from mobile if you want to
I would say you can have a team that works on the Form side of things for both Web and Mobile - They could go with one form for all approach
But then if there is an absolute need for separate form for mobile, you can have two teams working on two different forms and it will still work but i would recommend the one form approach.

Ouch.. I missed many update for MS CRM/365, then how to maintain the source code if I work with separate team of developer? 1st team is focused for web desktop form and 2nd team is focused for tablet?

You do not have to create a seperate form for tablet and phone anymore.
With Unified Interface, your main form is rendered on all devices, as long as you enable for Mobile in your entity configuration page. This enables you to customise once and deploy to any device

If this is a custom entity you are creating thats not already displayed in Dynamics 365 mobile app, you will have to add the new entity to the App, Please see below how you can add it to the App
