Two activities, two emails

I posted something earlier about how CRM is creating two activities when I enter a new phone call.  Also when I click an email address from an account record in order to send an email two new emails windows are created.  Is CRM seeing two instances of Outlook for some reason?

    Thank you Aaron for looking into this.  Yes it is only occurring from within the CRM Outlook client.  I did install the CRM 2013 Outlook Client update that came out after the October Roll up.  I was getting two email windows all the time before that however scheduling two activities starting occurring after the update.  

    Also when I enter a phone call from Activites in the Account Summary screen it says Customer not found.  This was not happening prior to installing the update either.  CRM allows me to close the call but when you view Completed Calls the contact name is no longer listed but the account is.
