I have the setup complete, and the report will print in word with one exception. There is no data, only shaded boxes and the logo. Any ideas as to why this would be happening?

Have you tried re-exporting the XML from the Standard Report - then delete what's attached to your Word Template and attach the new XML. Is the standard report heavily modified?
If all else fails, I would go back to first principles. Start with a modified SOP blank form in the reports dictionary....with no changes form the original. Then open a new word template and print...you should get data. then start to modify the template / standard form [remembering to refresh the XML] and take it step by step from there.
