2014 W-2 Additional Medicare Tax in wrong Box#

When I ran the W-2 calculation and printed the draft 2014 W-2, the Additional Medicare Tax is included in Box 4 not in Box 6.  Does anyone know why this occurred? 

Also, I'm having an issue with a small amount (as in cents per month) of taxable earnings being recorded during each payroll processed both in 2014 and now in 2015.  Any clues about why or how to stop it from continuing to occur?

    That happened to me last year (incorrect box) and I was missing a payroll update.

    There are 2 reasons for the additional medicate not being properly displayed on the W2s.

    1.  In Deduction History>fed9 2014, the W2 control box is not set to "Additional" Medicare.

    2. You don't have updates installed.

     

    As for the fed9 calculating small amts,  we are seeing for some people.   I believe that if you will add a Min Subjwage = 200000 and Max Subjwage = 9999999 on the Deduction Tab of Deduction Maintenance, this will stop.  We removed that setting when we added the exemption based upon Fed6 but for some people , it appears that we  still need it.
