Word Invoice Template - Emailing a 2nd page or document if a certain condition exists

We are trying to add a second page to a SOP Blank Invoice Form if a certain condition exists,  (If the balance due is greater than zero, for example).     I'm pretty sure this will take some programming to get it to work, if it can at all.     We are trying to send a "Credit Card Authorization" document as a second page or second attachment with the invoice.    It is a very simple Word Document, but I can't get it to work without the headers/footers appearing on the second page.   If anyone has any ideas or can do the custom work, please advise.
