Hi,
How I can attach a file to form in dynamic 365.
I could not find the Notes button in form.

If note is not available in the form then go Form and add notes from Insert tab.

Make sure that the Notes has been enabled for the entity that you want to add attachments to.
Go to Settings -> Customize the System (or your solution).
In the solution, Expand Entities, and Navigate to the entity where you want to add the Notes to.
In the section Communication & Collaboration, check the "Notes (including attachments) checkbox.
Save and Publish the Entity changes.
Go the the Form of that particular entity. Click on the Insert Tab, and click on the Note icon to insert the Social Pane (Notes Pane) to your form.
