Microsoft flow-power automate-customer creation-Microsoft forms-approval flow template not working

I tried to test customer creation with custom fields workflow approval template in power automate. But I got this error Action 'Create_record_(V3)' failed. once approved it does not update the customer in BC . For the standard fields this workflow is working .
