A strange behavior we are facing in our Production environment.
Two forms are getting merged and showing as a single form in CRM.
Eg: it is showing Form Name as : Phone Call(ABC) but the contents are form phone call(CDE) form
We thought it as cache issue but clearing cache, restating is not helping
Desktop Type: VDI
CRM: Dynamics 365 - V9 (On premise)

Are you moving the forms using solutions? Are they managed or unmanaged solutions? It sounds like your moving unamnaged solutions. My fear here is that if you looked in customization.xml file of your solution I'd suspect that your GUID of the new form is the same as the existing in production.
