What is the process of Upgrade dynamic CRM 2011 Onpremise to D365 onpremise. Is there any documentation for it. Please share the same.

You can’t directly upgrade from Microsoft Dynamics CRM 2013 or Microsoft Dynamics CRM 2011 to Microsoft Dynamics CRM 2016. However, you can upgrade a Microsoft Dynamics CRM 2013 or Microsoft Dynamics CRM 2011 deployment in-place by using a valid Microsoft Dynamics CRM product key, or migrate by using a Microsoft Dynamics CRM 2013 trial product key, as an interim step in the upgrade process.
Initially, you must include the preliminary task to migrate the CRM 2011 organizations to CRM 2013, from 2013 you need to upgrade to 2015 and then from 2015 to Dynamics 2016

I agree with Akash, you will have to gradually upgrade Dynamics CRM 2011 to the major builds of 2013, 2015, 2016 and finally version 9.
The safest and recommended approach would be to do this upgrade on an another CRM 2011 server which would have the latest 2011 CU update applied and then you would go through by importing the DB of the existing deployment to this new deployment. Post this activity, you can slowly move to 2013, 2015, 2016 server major builds and so on through server update installations.

From CRM2011 you can go to CRM2013SP1 in one hop ... make sure to run code comaptibility checks all through as Javascript in partilcualr has to get changed between 2011 and 2013 and again for D365v9 - if you don't intend to actually deploy at 2013 then just one change of course
