    Hi Jay2014

My name is Andres Guzman, I'm glad to help you

I recommend you to take the traces when you reproduce the error message.

You can take this info using the next tool: crmdiagtool2011.codeplex.com

I don't know the version of your CRM Server but this tool works with 2011, 2013 and 2015.

Please try to find the error using a search in each file and share with us to validate additional info. Please remove all the info related with your internal info.

    Regards,

If you are online that will be harder to get more details but sometimes you can find under the system jobs if it is related to an async operation

    If you are on-premises

    you can enable trace

    support.microsoft.com/.../907490

that will help you find more details

it will generate a lot of hard-to-read files. use this tool to spot the error detail faster

Just creating a new entity usually doesn't break like this until something like a relationship between that and something else is corrupt or unpublished.

was there any lookup or relationship added and removed or saved but not published?

Try to re publish changes. Do an IIS reset. Clear all browser cache.

    Then test it.

check any plugin/workflow or script causing the issue. try to disable any plugin/script if it exits and check whether it works or not

Thank you all for your help ! I removed all of the fields from the Form and re-published it and it worked. I will take it from here and investigate further.
