Dynamics CRM automatically creates duplicate Opportunities

This happened today.  One of the team closed one opportunity and it shows up in the Timeline 12 times.  Any idea why this may happen?

    Hi Mary,

    Please check if any workflow/plugin/flow was running at the moment the duplicate opportunity was created.

    You can check workflow in System Jobs.

    Check plugin in Plug-In Trace Log.

    Look at the "Create By" field of Opportunities to see who created it, and then check the run history of all flows for this user.
