We have been experiencing coverage groups changing per item on a random basis. Is there a setting anywhere that allows this or is it a bug?

This is abnormal behavior, it may be related to internal customization or bug in your application
Can you provide an example of what you mean? You can configure it to have different coverage groups by item by dimension in the item coverage setup for example, so a different logic is applied at warehouse A to B for example.
