Is there a way to make or add new field in Notes section where the title is lookup field, so that user can select the value from the lookup field and enter some texts under notes and once 'Done' is clicked, it saves the new notes with selected lookup value as title.

The notes entity is not customizeable. The only way I see this happening is to define a custom entity to replace the notes behavior.
