Can anyone guide me how to upgrade from Dynamics AX 2012 RTM to AX2012 R3 Upgrade. If required kindly share me links or documents.

You can follow this link to perform an in-place upgrade. This outlines all the necessary steps needed for the upgrade.

I need to upgrade into new server not in -place upgrade.
The process is still quite the same. You need to run the code and data upgrade in some system.
After that, you can copy the databases (business data + model database) in your new environment.
You can start with code upgrade to begin with in an isolated environment. Then you can go for a data upgrade in the same environment.
For Prod, you can setup new environments, move the codebase (model DB) and can do the final data upgrade.
