Auto posts on cases sometimes not created

What could be possible causes for auto posts to intermittently failed or not created.
Most of the time, it works fine. But when it occurs, like all cases created in a day or two won't have auto posts. Then it's fine back again. 
 
There is really no auto post record in the post table and nothing shows up in the timeline for the affected cases.
