Users unable to see fields on custom form

I've created a custom entity, with a then a custom form and custom fields. 

As the administrator, I have published these and can see them in my menu / view the list and open the form with all the details. 

I've added the custom entity to the permissions of the security role of another user (giving them organisation level access). 

There is no security on any of the fields in the form.

When the user logins into Dynamics, they can see the menu, get to the list view, but if they click on any of the items in the list or create their own new one - they can only see two fields...

I am not sure what else to do make this available for others to use within the organisation?

    Hello Bianca,

    If you open the form and click on "enable security roles" which option has been selected ? I created a custom entity and selected option "display to everyone" and it's showing up for another users -

    I made this available to a particular group, but just to be sure I have now updated that to be everyone.  The only fields that the user can see are the mandatory fields...none of the other fields.
