How can I successfully create a checkbox options in my Marketing 365 Forms?
I tried using this outsourced code, but somehow it pop out an error after a submission.

Thank you for the advice.
However if I'm looking for more than 2 options, is there any HTML i can use?
I tried another form field in Marketing 365 - "Multi-select option" but it does not allow me to save it because the option for the Contact Mapping/Lead Mapping is empty. This is because there is no suggested selections except " --Select--". (As per below)
As for the "Two Options" field, I can create it, but I'm unable to get the 2nd option.
