URL in E-mail notification - Document Approval/Workflow

When a PQ/PO is processed and sent for approval, the notification E-mail contains a URL that is supposed to navigate the approver to the particular PQ/PO. The URL is navigating the user to an invalid page/old instance (see attached)
 
How can we update the URL to point to the correct location?
