Word Template Checkbox check on Word Doc Issue

I have a option set field in CRM with 5 options
 
options- ab, cd, ef, gh, ij
 
User selects one option as /ab/ in CRM.

Now in word template i have taken 5 checkboxes. Now I want to attach the CRM value to checkboxes in word. Do anyone has idea, like if in CRM /ab /  is selected then in my Word checkbox of /ab/ gets checked.
