
Is there a way to create a CASE template or workflow in CRM 4.0 so certain fields can be auto populated for a new case? Same customer, case type, product, etc.

You have various option. Here are two ideas:
a. On the case entity, set the case type to have a default and other picklists too.
b. You could create a workflwo 'copy case' that would create a new case adn copy certain fields from the existing case such as customer, case type, product, etc. So if a customer calls in adn has same issue as before, with two clicks you create a new case with all those settings.
