how did you turn of the New Relic Agent? We are also facing this problem and don't see a resolution other than the one you posted.

We fixed this issue with Microsoft Support. We found that New Relic Agent was adding monitoring Script (JS files) into page header, which was causing problem while loading the .srcmap files. After turning off New Relic monitoring, CRM pages started working normally.
We should turn off all such monitoring tools which adds Scripts to CRM page header to load them properly.

Same issue in our organization too. The version we are using is Microsoft Dynamics CRM 2016 (8.1.0.359) (DB 8.1.0.359).

Same issue in our organization on Dynamics 365 v.1612(8.2.1.176) on-premise.
Multiple HTTP404 .srcmap reports on every load that severly affects loading time and end user experience.
We are experiencing this issue as well. No CRM form is loading. We are having D365 on premise version 8.2.0.749.
Is anyone already reached to Microsoft Support?
