When you modify an Extender field, do you need to 'refresh' the SmartList somehow, such that the SmartLists that contain the Ext field will show the updates?

If you mean modifying the data, no. Each time a SmartList runs it reads the data tables. If, however, you are modifying the extender window (such as adding selections to a pull down list or adding fields) you need to refresh the view. You need to sign in to GP as SA to be able to refresh Extender views.
Can you point me to the navigation for refreshing? Thanks!
Login to GP as sa. Open Extender. Open the window. Make a change. Save the window. You will be asked if the view should be updated.
Sorry, I do not get this message when I save the window, logged in as SA! Is there a manual refresh by chance?
Login to GP as sa, open Extender, click on Views, and double click the view that needs to be refreshed. This should do the trick.
I double click but still don't get any update message! Is there another refresh routine?
Try opening Smartlist Builder, select the smartlist you are trying to update and double click the view and then resave it.
