Custom Entity Quick Create form Not showing

Hello,

I have created a new Entity and associated an QuickCreate Form on it.

When I want to quick create an entity, the first time I can see the fields as expected. But if I retry a second time, then the fields are not visible anymore.

Is there any known bug?

FYI:

CRM Dynamics 365 on premise version 1612

Chrome Version 65.0.3325.181

Windows 7 entreprise

At the first try I can see my fields and create my custom entity:

If I then cancel and want to recreate one clicking on the + quick create button of the ribbon I get:

    Hi ,

    Which  browser are you using ?

    This issue is particularly happening on chrome browser . Many user is facing this issue . Please  refer below thread -

    In addition follow the recommendation of above thread and if it does not help then wait for Microsoft to fix the issue or raise a ticket to Microsoft.
