Duplicate Primary Key entries in SOP

Hi All,

I am hoping someone can help here. We are having an issue at the moment where we are getting frequent duplicate primary key errors in our system. We are using GP 2013. I have asked one of the technical guys to collate the records. See below the problem invoices.. Does anyone know what would cause GP to give this duplication?
