So if I imported a large list of opportunities and then a month later I had some changes to some of the fields and I imported the list again.... What uniquely identifies each opportunity so that it updates and doesn't create a duplicate record?

Prashant has a great answer but sometimes you need to integrate with external systems and have the updates flow into CRM. CRM provides a number of nice capabilities to support this.

When you want to update data of opportunities;
You need to export existing list of opportunities from CRM, update it and then reimport.
In the export list, the first three columns are hidden and they carry uniqueness of each record.
