Error on converting phonecall to opportunity

Hi there,

On the phone call form if I convert phonecall to an opportunity I get this error.

TypeError: Cannot read property 'getValue' of null at e.convertActivityClick (testdev.crm4.dynamics.com/.../SalesCommandBarActions.js:2:28305) at i.convertActivityClick (testdev.crm4.dynamics.com/.../SalesCommandBarActions.js:2:18063) at e.executeFunction (test04dev.crm4.dynamics.com/.../app.js at e.execute (test04dev.crm4.dynamics.com/.../app.js at e._executeIndividualEvent (test04dev.crm4.dynamics.com/.../app.js at e._executeEventHandler (test4dev.crm4.dynamics.com/.../app.js at Object.execute (test04dev.crm4.dynamics.com/.../app.js at e._executeSyncAction (test04dev.crm4.dynamics.com/.../app.js at e._executeSync (testdev.crm4.dynamics.com/.../app.js at e.executeAction (https://testdev.crm4.dynamics.com/uclient/scripts/app.js?v=1.3.1916-190625-161532:443:236)

On investigating SalesCommandBarActions.js below line is causing the error

(d = a.data.attributes.get(t.MetadataDrivenDialogConstants.Subject).getValue().toString())

How can i resolve this issue or is there any way i can edit SalesCommandBarActions.js file to fix it?
