Custom Survey variable missing in D365 Send Survey form

I have 4 custom variables on a survey...one of them called ActionOrCaseNumber is not displaying in D365 when we use the native Send Survey button. On the survey design it is the last variable. In D365 it just does not show. The vertical scroll bar just ends at the variable above it. Looking for help figuring out why a survey variable may not display!!  
 
 
send survey in d365 ends up like this. Notice locale is prompt although it somehow got removed from list of variables above.

    I noticed this in 2 surveys actually. In both surveys the Locale variable is missing on the personalization screen as if someone removed it. It shows up in the list of variables in D365 when "send survey" is pressed but it has no values to choose from. I really don't want to recreate these surveys as there are custom flows that involve them.
