Embed Customer Voice Form in Dynamics entity form as a button

Hi All,

I have a requirement to embed the customer voice survey form inside a dynamics contact entity form as a button on the ribbon. When user clicks that button, there will be a pop-up or new tab that opens with the survey questions. Can anyone help me with the steps to implement this?
