I have exported a Dynamic Worksheet from a system view in CRM by using EXPORT TO EXCEL --> Dynamic Worksheet from the available options. The workbook is saved to the local C:/ drive on my machine.
On selecting the Refresh from CRM option in the Data tab of the Ribbon, nothing happens - the data does not refresh. The Outlook for CRM add-in is installed on the machine and configured properly.
Upon selecting the Refresh All button adjacent, the following error message is written to the cells:
JavaScript required
JavaScript is required. This web browser does not support JavaScript or JavaScript in this web browser is not enabled.
To find out if your web browser supports JavaScript or to enable JavaScript, see web browser help.

The following KB article: https://support.microsoft.com/en-gb/kb/2556284, outlines a similar problem, with the exception that I do see the correct data on the file's first open. I can painfully work around the problem by using the advice outlined under the heading More Information.

The answer from Loz did the trick for us.
Configure your Outlook Client using the External CRM URL. Then you should be able to use the Refresh from Dynamics 365 button in Excel to refresh the data (rather than the Refresh All button).

Thanks for the help Drew. Unfortunately, this did not work and all of the settings in the guide are already configured here.
After some more investigation, it looks like the connection string in Excel file is using our external facing URL rather than the internal one and because of this, the credentials are not being passed across automatically.

Only suggestion I have (outside of opening a ticket with Microsoft) because I do see this is an issue that is out there that is noted in a number of blogs and with Microsoft's workarounds but I did see the following article that discusses the automatic login to CRM as part of the setup for dynamic worksheet.

I am wondering if you can configure this on one of the machines and see if it gets past the problem. Sorry if this does not help but thought it would be worth a try.

Hi Drew,
1. It is CRM 2016 On Premise with IFD configuration.
2. This is not working for any other user that I know of. Across numerous machines I have tested, this feature is behaving the same.
3. The Outlook client is working, we track 100's of emails every day and they all appear to be in the system.
4. I have not tried reinstalling the CRM Outlook client; that is something I can do, but I assume it would mean that the installation is flawed on multiple machines.

1. CRM 2016 On Premise with IFD configuration or CRM Online?
2. Is this working for any other users in your organization or not working for anyone or is the issue tied to this machine?
3. Have you checked if the Outlook client is working at all? Can you track emails, is it showing contacts that are tracked in CRM, etc.? You had mentioned it was properly configured so maybe the answer is "yes" here.
4. Have you tried to reinstall the CRM Outlook client?

The operating system is Windows 10 on the latest stable build, the version of Outlook and Excel is Office 2016 Professional, and the browser typically integrated with both applications is Internet Explorer. We have set Microsoft Edge to redirect to Internet Explorer when using Dynamics CRM.
I have checked my security settings, and scripts including .NET, ActiveX, and JavaScript is enabled (should only need the JavaScript). I have used: isjavascriptenabled.com to test JavaScript in the browser.
