Hello,
I followed the Microsoft document instructions to attach name and email to Customer Voice when it is sent out by Customer Journey. The FLOW is successful, and the respondents appear in Forms Pro Responses, but not Customer Voice Responses. Is there a system bug?

Is Tammy able to see her name in the survey?
I have little experience with Customer Voice, and I just ran a test:
All my test survey recipients were not identified, therefore, they are all in anonymous status.
From your screenshot, I can see Joe has been recognized, did he also open the survey via marketing email?
While in marketing application, survey responses are synced properly from Customer Voice and corresponding contacts are associated to responses.
You could check whether any setting to identify respondents was set properly in Customer Voice, or open a thread in Customer Voice forum at same time.
