Errors logging into SL Web Apps - user name or password provided is incorrect

We are setting up Web Apps on our new SL 2011 SP 2 installation.

We are able to load the Web login screen and choose a company.  However, ALL users receive the message "The user name or password provided is incorrect"

I have added our windows logins to the employee set up in project.

What am I missing?  I have not set up anything in the SL application for Trusted Web Service Account Maintenance or Web Service look up security.  When I attempt to add to the Trusted Web Service Account maintenance, I get a SQL Server Message 911 Database '' does not exist.    The problem seems to be with the sp_addrolemember stored proc.

    Thank you, Mark.  We are using Windows Authentication.  The only blank entry in the domain table is for Master login password.  Does this need to be deleted?  I have always seen it, so I thought it was normal.

    Kathryn,

    Assuming your SL data bases are setup for windows authentication, check the domain table in SQL.  There is likely a record in there with a blank value in the databasename column.   Delete this record and then try the trusted web service account maintenance screen again.  Once you get your app pool user to save in that screen OK, the "username or password" error should go away.
